If you suspect that a particular symptom you have experienced is related to taking/applying the medication you are using, please follow these steps:

1. Re-read the Patient Information Leaflet (PIL) of the medication, as it contains information on expected adverse reactions and recommendations on how to proceed in case of serious side effects.

2. Consult your doctor or pharmacist, as they can provide professional advice on whether to continue, discontinue, change, reduce the dosage, or proceed with the treatment of the medication suspected of causing the adverse reaction.

3. Report your concerns by filling out the form below and/or submitting an online report to MALMED through the patient Adverse Reaction Reporting application.

Your report on suspected adverse symptoms/reactions caused by medication use can help reduce their frequency or prevent them altogether. You may report an adverse reaction that you have personally experienced, or one experienced by your child or a person under your care.

For a more thorough evaluation of cases and to draw relevant conclusions, please provide as much detail as possible in your adverse reaction report. If additional information is required to assess the reported case, we may need to contact you. Therefore, the following fields are mandatory in the report: Contact phone number and email address.

INFORMATION ABOUT THE PERSON WHO EXPERIENCED THE ADVERSE DRUG REACTION
